Options to apply:

1. You can mail a completed copy of this form along with a photo copy of your ID to us at 2201 Laverne Ave. Klamath Falls, OR 97603. 

2. You can email the completed form along with a copy of your ID to our Utility Billing Clerk at kate@sssd.org

3. You can stop by our office at 2201 Laverne Ave. to fill out the form and present your ID
